IMPORTANT Information for scheduling:
- Established Patient: A patient who has been seen in our office within the last 3 years.
- Follow-Up: For established patients receiving continued care for a previously treated condition.
- New Patient: A patient who has never been seen in our office or has not visited in over 3 years.
- Skin Check: A comprehensive, head-to-toe skin evaluation. Recommended to have at least once a year.
***Please remember to bring your photo identification and health insurance card to the appointment. All copays are due at the time of check out. All appointments require 24 hour notice to cancel or reschedule an upcoming appointment, if cancelled or rescheduled less than 24 hours a $50 fee will be assessed to patient.***
